Generative AI Knowledge Assistant
RAG-based clinical-documentation assistant with HIPAA-compliant audit trail.
Key results
- Documentation time -38%
- Clinician satisfaction +42%
- Audit-ready 100%
Context
A multi-specialty healthcare provider ran into the familiar problem of clinicians spending significant portions of every shift on documentation rather than patient care. Generic generative AI tools were not usable because of HIPAA constraints on sending PHI to external APIs.
Challenge
The solution had to run without sending PHI to external vendors, had to ground answers in the client's own clinical documentation and protocols rather than generic medical text, and had to produce a full audit trail for compliance review.
Approach
Thoughtwave deployed a self-hosted RAG-based clinical assistant: an open-weight model served locally, retrieval grounded in the client's documentation repository, PII/PHI redaction pre-prompt, and full audit logging retained per HIPAA books-and-records obligations. The 10-week engagement covered discovery, architecture, build, and a pilot cohort evaluation.
Outcomes
Documentation time dropped 38% for the pilot clinician cohort; clinician satisfaction with the documentation workflow rose 42%; audit reviews confirmed 100% compliance on every sample pulled. Rollout expanded to additional specialties in the following quarter.
